Health Management

As a medical model senior day community, Day-Break at Farmington meets National and Connecticut state guidelines for delivering health, rehabilitative, recreation and social services to individuals who need some assistance. By providing this support, we help our members maintain their health and independence, staying where they feel most comfortable—at home.
Day-Break’s individualized plan of care allows our staff to focus on each participant's needs and address them effectively through safe, reliable on-site care including:

Medical Care
We administer medications, monitor vitals and deliver follow-up wound care to maintain your health.

Health management and monitoring
We manage and monitor Alzheimer's, diabetes, hypertension, muscle impairments, cognition and more. We also provide guidance to help you understand any health concerns you might have, as well as maintain your health.

Coordination with personal physicians
We regularly communicate with your personal physicians to help you stay on top of your health.

Everyday personal care
If you need support with personal care, we’re here to help. Some of our members even shower here, so they return home well-cared for, refreshed and ready to enjoy their evening.

Social services
Our goal is to enhance our members' lives by providing support with the medical, financial and emotional ups and downs of everyday life. Bottom line: We want you to know we’re here for you. We’re happy to provide referrals for outside resources, support groups or financial assistance whenever necessary.

Social interaction, including a variety of activities and even enjoying meals together, helps keep our members well—physically and mentally. That’s why Day-Break provides:

Recreational programs
Our programs enhance physical and mental abilities. They include:

Nutritious meals
Day-Break participates in the Connecticut Adult and Child Food Program (CACFP), which has a rigorous set of guidelines for balanced diet and nutrition. We provide breakfast and lunch, as well as a snack each day, and we also customize meals for specialty diets.
Transportation
Convenient pick-up and drop-off
As part of our commitment to put the needs of our members and their caregivers first, we offer flexible morning pick-up and afternoon drop-off times. Transportation is handicap-accessible, and travels to and from our facility, right from our members’ homes.
